Thank you for choosing Fresno Breast Surgery! We aim to provide a quiet, safe and calming atmosphere for our patients and their families. We look forward to seeing you and hope the following information will help answer any questions you may have about being our patient. If you have any questions that aren’t addressed here, please let us know.

 

For Your First Appointment

 

New Patient Questionnaire

All new patients are asked to fill out a questionnaire regarding your personal and family health history. You may download a copy here or request a copy to be mailed to you to prior to your appointment. Please fill out your questionnaire at home and bring with you at the time of your appointment. If you have not obtained your paperwork, please arrive to the office 30 minutes prior to your appointment to complete. You may need to ask your family members about their health history, especially about your family history of cancer.  Please answer all questions as completely and thoroughly as possible. If you do not know the answers to all the questions, that is ok as well. Please use blue or black ink.

 

Medication and Pharmacy Information

Please bring a complete, detailed list of your medications, including prescription and over the counter medications as well as herbal and vitamin supplements. Please provide the name, phone and fax number for your preferred pharmacy.

 

Imaging Studies

Dr. Hadcock will review your imaging studies during your consultation to determine an appropriate treatment plan. Please bring all current films and imaging reports as well as all films and reports done at least 2 years prior to your appointment. We are happy to request films and arrange to have them delivered to our office one and half weeks prior to your appointment with us. Patients from out of town or seeing us on short notice may need to hand carry their films and reports. If you are hand carrying your own films, please feel free to check the film jacket to make sure it contains all needed films and reports. Images must be in printed film format, CDs provide inferior quality images.

 

Health Insurance

Please bring your photo ID and current insurance cards, as well as any required co-pays. While we are not providers for all health plans, we can often find a way to work with patients with special circumstances, please let us know if you would like to discuss details.

 

Support and Questions

We’re going to cover a lot of information at your first visit! Please feel free to bring someone close to you to your appointment for support, as well as a list of questions for the doctor. It can be hard to remember all of your concerns and questions during your appointment. An extra pair of eyes and ears and a list of questions can help make sure you have a productive visit.

What to Expect at your First Appointment

After checking in at the front desk, you will be escorted to an exam room and Cindy will obtain your vital signs and review your medical history, medications and prepare you for your consult and examination with Dr. Hadcock. Dr. Hadcock will discuss your concerns, review your records and perform a thorough physical examination. This complete and thorough assessment of your overall health allows us to safely assess and prepare you for all upcoming treatments, including anesthetic and surgical procedures. Your privacy and comfort will be respected at all times, with gowns and drapes provided for your examination. After your evaluation, you may not need further followup care with us, or you may need additional diagnostic testing, a revised follow-up plan or surgery. Dr. Hadcock will discuss her recommendations and together we will make arrangements for the next steps in your care plan.

Appointments

Situations can arise during the day where a patient may need additional time with the doctor to discuss complex treatment options and issues. While we make every effort to stay on a timely schedule, these situations can occur with any patient at any stage in their treatment. We appreciate your patience and understanding in accommodating these events just as our other patients will be patient and understanding should you need extra time with the doctor in the future. We will advise you of any significant delays when you check in for your appointment, please let us know if you have any specific scheduling needs or concerns and we will make every effort to accommodate you and be respectful of your time.

 

Physical Examinations & History

Physical examination and discussion of your medical history are an important part of assessing your medical and personal needs. All examinations are done with the utmost respect to your comfort and privacy and gowns and drapes are provided to accommodate your examination. All questions and discussions about your condition and concerns are so that we can provide the best care possible.

 

Office Procedures

Your evaluation may include minor office procedures such as needle biopsies or needle aspirations. We will sometimes perform other minor procedures under local anesthesia. These procedures are done by sight, feel and/or ultrasound and are generally quite safe. Complications are rare and are managed promptly and appropriately when they do happen. Please let us know if you have any questions or concerns.

 

Questions for the Doctor

If you think of questions after your visit, please feel free to call us or drop them by the office. Our friendly staff will relay them to the doctor to get back to you.
