*Reproductive and hormonal risk factors include menarche at age 21 years or earlier, menopause at age 55 years or later, nulliparity, first full term pregnancy after 30 years, current use of postmenopausal hormone replacement therapy.
**Weak family history is defined as two or fewer second-degree or more distant relatives with postmenopausal breast cancer.
Strong family history is defined as any first-degree relative with breast cancer, three or more relatives with breast cancer, or any second-degree relative with breast cancer before 40 years.
Very strong family history is defined as two or more first-degree relatives with breast cancer or ovarian cancer, one or more first-degree relatives with breast cancer before age 40 years, or any first-degree relative with bilateral premenopausal breast cancer.
