Patient Questionnaire

In preparation for your visit, we ask you to complete a questionnaire regarding your health history and that of your family. If there is time in advance of your appointment, this will be sent to you in the mail to complete at home and bring with you to your appointment If not, please arrive 30 minutes early in order to answer these health questions. Please bring a complete list of your medications, both prescriptions and over the counter and herbal medications. Providing your pharmacy phone number and fax will assist us in prescribing drugs safely and efficiently for you.

The questions regarding your history and medications, and a detailed examination allow us to safely assess your needs especially in planning for a smooth and safe anesthetic and surgical procedure if necessary. It is normal for the doctor to examine you as thoroughly as necessary to address your particular problem. This includes an overview of your entire general health and physical examination. We will respect your privacy and you will be appropriately dressed in gown or drapes to accommodate the examination.

While we make every effort to stay on a timely schedule, situations do arise during the day when a particular patient needs additional time. We appreciate your patience and we will make every effort to be sensitive to your needs.
